GUIDELINES FOR FILLING UP THE APPLICATION FORM-6 General Instructions Who can file Form-6 1. First time applicant on attaining age of 18years or more on the first day of January of the year with reference to which the electoral roll is being revised. 2. Person shifting his / her place of ordinary residence outside the constituency in which he / she is already registered. When Form-6 can be filed 1. The application can be filed after draft publication of electoral roll of the constituency. The application is to be filed within the specific days provided for the purpose. Due publicity is given about the above period when the revision programme is announced. 2. Only one copy of the application is to be filed. 3. Application for inclusion of name can be filed through out the year even when the revision programme is not going on. During non-revision period, application must be filed in duplicate. Where to file Form-6 1. During revision period, the application can be filed at the designated locations where the draft electoral roll is displayed (mostly polling station locations) as well as the Electoral Registration Officer and Assistant Electoral Registration Officer of the constituency. 2. During other period of the year when revision programme is not going on, the application can be filed only with the Electoral Registration Officer. How to Fill the Form-6 1. The application should be addressed to the Electoral Registration Officer of the constituency in which you seek registration. The name of the constituency should be mentioned in the blank space. 2. Name (With Documentary Proof) The name as it should appear in the electoral roll and Electors Photo Identity Card (EPIC) should be furnished. The full name except the surname should be written in the first box and surname should be written in the second box. In case you do not have a surname, just write the given name. Caste should not be mentioned except where the caste name is used as part of the elector’s name or a surname. Honorific appellations like Shri, Smt. Kumari, Khan, Begum, Pandit etc. should not be mentioned. 3. Age (With Documentary Proof) The age of the applicant should be eighteen or more on 1st January of the year with reference to which the electoral roll is being revised. The age should be indicated in years and months. e.g. A person born on or upto 1/1/1991 will be eligible for inclusion in the electoral roll which is being revised with reference to 1/1/2009. Persons born on 2/1/1991 or thereafter upto 1/1/1992 shall be eligible for inclusion during the next revision with reference to 1/1/2010. 4. Sex Write your sex in full in the space provided e.g. Male / Female. Eunuchs have to choose from one of these options. 5. Date of Birth (With Documentary Proof) Fill up the date of birth in figures in the space provided in dd/mm/yyyy. Proof of date of birth to be attached are as under: (i) Birth certificate issued by a Municipal Authorities or district office of the Registrar of Births & Deaths or Baptism certificate; or (ii) Birth certificate from the school (Govt. / Recognised) last attended by the applicant or any other recognised educational institution; or (iii) Illiterate or semi-illiterate applicant who is not in possession of any of the above document are required to attach a declaration in prescribed format by either of the parents already included in the electoral roll in support of the applicants age. The format will be supplied on demand. N.B. In the case of applicants born on or after 26.01.1989, only birth certificate issued by the Municipal Authorities or district office of the Registrar of Births & Deaths is acceptable. 6. Place of Birth In case born in India, please mention name of place like Village / Town, District, State. 7. Relation’s Name: In case of unmarried female applicant, name of Father / Mother is to be mentioned. In case of married female applicant, name of Husband is to be mentioned. Strike out the inapplicable options in the column. 8. Place of Ordinary Residence Fill up the full and complete postal address including PIN code where you are ordinarily residing and want to get registered, in the space provided. Proof of ordinary residence to be attached are as under: (i) Bank / Kisan / Post Office current Pass Book, or (ii) Applicants Ration Card / Passport / Driving License / Income Tax Assessment Order, or (iii) Latest Water / Telephone / Electricity / Gas Connection Bill for that address, either in the name of the applicant or that of his / her immediate relation like parents etc., or (iv) Postal department’s posts received / delivered in the applicant’s name at the given address. NOTE: If any applicant submits only ration card as proof of address, it should be accompanied by one more proof of address out of the above categories. 9. Details of Family Members Already Included in the Electoral Roll Please fill up name and other particulars of immediate family members i.e. Father / Mother / Brother / Sister / Spouse included in the current electoral roll of the constituency. Name of any other relation like uncle, aunt, cousin brother / sisters etc. not to be mentioned. 10. Declaration Please indicate date from which you are residing in the given address. In case the exact date is not known, fill-up month and year. If your name is already included in the electoral roll of any other constituency, please write legibly the full previous address with PIN code. If you already have been issued with a Photo Identity Card by the Election Commission, please mention the card number (printed on the front side) and date of issue (printed on the back side) of the card in the space provided. Please attach a self-attested photocopy of both sides of the card. Miscellaneous In many places the photograph of the elector is also printed in the electoral roll. You have the option to submit one recent coloured passport-size photograph alongwith the form. The photograph will be used to print your image in the electoral roll and issue of identity card, if required. Please also give your full name and address on the acknowledgement and intimation portions. Please give your mobile number and E-mail I.D in the Form , which is optional , as the same ,if given, may be used by the Electoral Registration Officer for further communication with the applicant whenever required.

Guidelines of the Scheme “Entrepreneurship Development Programmes ( IMC / EDP/ ESDP / MDP)” under “Promotional Services Institutions and Programmes”
A. Background
The Micro, Small and Medium Enterprises (MSMEs) sector has emerged as a highly vibrant and dynamic sector of the Indian economy over the last five decades. MSMEs not only play crucial role in providing large employment opportunities at comparatively lower capital cost than large industries but also help in industrialization of rural & backward areas, thereby, reducing regional imbalances, assuring more equitable distribution of national income and wealth. MSMEs are complementary to large industries as ancillary units and contribute enormously to the socio-economic development of the country.
The Ministry of Micro, Small and Medium Enterprises (M/o MSME) promotes the development of micro and small enterprises in the country with the objective of creating self-employment opportunities and upgrading the relevant skills of existing and potential entrepreneurs. In order to promote establishment of new enterprises and creation of new entrepreneurs, M/o MSME has been implementing various schemes and programmes.
Entrepreneurship Development is one of the key elements for promotion of micro and small enterprises, particularly, the first generation entrepreneurs. Entrepreneurship, and resultant creation of employment and wealth, is a major mean for inclusive development. Hence, entrepreneurship development has been one of the priorities in countries, the world over.
The Office of DC (MSME) conducts a large number of vocational and entrepreneurship development programmes. While vocational training is implemented by various Departments of the Government, the responsibility of entrepreneurship development lies largely with this office. The Entrepreneurship Development Programmes (EDPs) are conducted through MSME-DIs, with focus on entrepreneurial development coupled with

specific skills relating to trades like electronics, electrical, food processing, etc, which enables the trainees to start their own ventures. The programmes covered include the following:
1. Industrial Motivation Campaigns (IMCs)
2. Entrepreneurship Development Programmes (EDPs)
3 Entrepreneurship Skill Development Programme (ESDPs)
4. Management Development Programmes (MDPs)
These programmes are of short duration and the curriculum is designed based on the needs of the industry and are customized, if required by the clients. 20% of the targeted EDP and ESDP of the duration of 2 weeks and 6 weeks respectively, are conducted exclusively for the weaker sections of the Society (SC /ST/Women/Physically Handicapped), for which no fee is charged. Besides, a stipend of Rs.125/-per week per candidate is provided.
The Scheme is an ongoing one and is being continued for the 11th Plan period.

The course contents of the Entrepreneurship Development Programmes are designed to provide useful information on product/process design, manufacturing practices involved, testing and quality control, selection and usage of appropriate machinery and equipments, project profile preparation, marketing avenues/techniques, product/service pricing, export opportunities, infrastructure facilities available, finance and financial institutions, cash flow, etc.
Courses conducted under ESDPs are in Machine Shop Practice, Heat Treatments, Electroplating, Sheet metal, Welding, Tool & Die Making, Glass & Ceramics, Industrial & Art Wares, Herbal Cosmetics, Fashion Garments, Hosiery, Food & Fruit Processing Industries, Information Technology, Hardware Maintenance, Soap and Detergents, Leather Products/Novelties, Servicing of Household Electrical Appliances and Electronic Gadgets, Gem Cutting & Polishing, Engineering Plastics, Tour operators, Mobile repairing, Beautician etc.

Courses conducted under MDPs are in Industrial Management, Human Resource Management, Marketing Management, Export Management & Documentation, Materials Management, Financial Management, Information Technology & Exports, ISO 9000, WTO, IPR etc.
5% of annual budget under the Scheme will be utilised for providing training aids, equipments and other related infrastructure to strengthen the quality of training activities in the MSME DIs and its Branch Institutes. Allocations under training aids will be demand based for items like LCD projectors, White Boards, P A system etc.
C. Procedure for selection of candidates
The applications are invited from the candidates through advertisement in Newspapers and websites of office of DC (MSME) as well as MSME-DIs. Selection Committees at MSME-DIs select the candidates taking into considerations their qualification, past experience and suitability for the course. The minimum age of the candidate is 18 years and there is no upper age limit. The qualification for the participant for a particular programme is mentioned in the advertisement taking into consideration the subject of the training programme. Preference is given to the candidates from weaker sections (SC/ST/Women/Physically Handicapped).
D. Implementation of the Scheme
The Scheme is implemented through MSME-DIs and Branch MSME-DIs. The process of implementation of the Scheme will be as follows:
i) In the beginning of every year tentative targets for the various training programmes under the scheme shall be allotted by the office of DC(MSME) to MSME-DIs as per norms decided on the basis of working age population of the state as well as staff strength and past performance of the MSME-DI. The targets may be further modified keeping in view the performance of the MSME-DI, local demand etc.
ii) Director of MSME-DIs shall decide topic, location, and schedule for conducting each training programme in consultation with the officers of MSME – DI and stakeholders.
iii) The complete training calendar for each MSME-DIs shall be uploaded in the website of each MSME-DIs in the beginning of the year in the following format.

iv) Director of the MSME-DIs shall nominate appropriate number of officers as nodal officers for conducting the training programmes.
v) Each nodal officer may be given the targets for conducting training programmes grouped on the basis of topic or location. The nodal officer shall plan in consultation with the Director, the venue, the target group and the dates for conducting the programs.
vi) The nodal officer shall also identify appropriate faculty/ guest speakers for each programme keeping in view the expertise required and available.
vii) Guest faculty means outside faculty other than MSME-DIs / Ministry of MSME officials with special knowledge in particular topics. The guest faculty will be provided honorarium for delivering the lecture / demonstration of Rs.500/- for the session of one and half hrs. for EDP/ESDP and IMC and Rs.700/- for MDP. A particular guest faculty cannot be given more than 30% of the total lectures in a programme. The faculty invited for inaugural / valedictory function will not be given honorarium.
viii) Course material shall be provided to the participants for their future reference.
ix) Stipend to the participants of stipendiary programmes: 20 % of the total targeted EDP/ESDPs will be conducted exclusively for weaker sections of the society i.e. (SC/ST/women and PH) with a stipend of Rs.125/- per week per candidate. The stipend will be given to those candidates who have at least 80% attendance and have been given certificate. Candidates absent during some of the training days, but otherwise eligible, will get pro-rata stipend.
x) The final Bills for the training programmes shall be submitted positively within 15 days of completion of the programmes.
x) In the website of every MSME-DI, a feedback window shall be opened, where the participants of the training programme and the other stakeholders may post their comments on the training programmes organised. The Director/Nodal Officer shall be responsible for monitoring the feedback received and take appropriate action on the same. An action taken report of the feedback received shall be forwarded by the

Directors of the Institutes to the Headquarter every month with the progress report of the training programme.
E. Monitoring/Evaluation
The progress on training programme will be monitored on monthly basis. Each MSME-DIs will send monthly progress report to Director/DD In-charge of Skill Development Programme in Office of DC(MSME) in the prescribed format and he will monitor/evaluate the progress / effectiveness of the training programmes.
F. Details of Training Programmes (IMC, EDP, ESDP & MDP )
MSME-DIs, organize number of training programmes to train potential entrepreneurs in improving their techno/managerial knowledge and skill with a view to facilitate them to establish MSEs in various fields. The training programmes conducted are:
1. Industrial Motivation Campaigns (IMCs)
2. Entrepreneurship Development Programmes (EDPs)
3 Entrepreneurship Skill Development Programmes (ESDPs)
4. Management Development Programmes (MDPs)
F.1.INDUSTRIAL MOTIVATION CAMPAIGNS
Industrial Motivation Campaigns are organized to identify and motivate traditional / non-traditional entrepreneurs having potential for setting up MSEs so as to lead them towards self-employment. The programme outlines are as follows -
1. AGENCY : These programmes are conducted by MSME- DIs.
2. DURATION : One day.
3. INTAKE CAPACITY: No limit.
4. TRAINING FEE: No Fee .
5. MAXIMUM EXPENDITURE: Rs.8000/-

Entrepreneurship Development Programmes are being organized to nurture the talent of youth by enlightening them on various aspects of industrial activity required for setting up MSEs. These EDPs are generally conducted in ITIs, Polytechnics and other technical institutions, where skill is available to motivate them towards self-employment.
20 % of the total targets of EDPs are conducted exclusively for weaker sections of the society i.e. (SC/ST/women and PH) with a stipend of Rs.125/- per week per candidate. No fee is charged from the SC, ST, women and PH candidates under these programmes.

Comprehensive training programmes are organized to upgrade skills of prospective entrepreneurs, existing workforce and also develop skills of new workers and technicians of

MSEs by organising various technical cum skill development training programmes with the basic objective of providing training for their skill up gradation and to equip them with better and improved technological skills of production. The specific tailor made programmes for the skill development of socially disadvantaged groups (OBC, SC, ST, Minorities and women ) are organized in various regions of the states , including the less developed areas. The target group for these programmes are SC, ST, OBC, women, minorities and other weaker sections.

The objective of imparting training on management practice system is to improve their decision-making capabilities resulting in higher productivity and profitability of existing & potential entrepreneurs and developing new enterprises. Inputs on a variety of topics of managerial functions are provided to the participants by experts, which aims at dissemination of knowledge of scientific/modern management techniques /practices.
Management Training course on various areas of industrial management are devised for owner-cum-manager and supervisory level personnel of small scale industries. These training programmes are designed keeping in view the demands of the area and the local requirements of the industries. The nature of target groups and its profile determines the course content to be made in order to make them suitable for contemporary managerial practices which may be used by MSMEs executives for attaining desired strength of managerial action. The various topics covered under these training programmes pertain to various Management functions like Industrial Management, Human Resource Management, Marketing Management, Export Management & Documentation, Materials Management, Financial Management, Information Technology & Exports, ISO 9000, WTO, IPR etc.

REPORT OF THE GOVERNING BODY ON
THE WORKING OF THE ASSOCIATION IN 2005-06
Friends,
We extend to you all a warm welcome to the 97th Annual General
Meeting of the Ramakrishna Mission. We are happy to place before you
a brief report of the activities of Ramakrishna Mission for the year
2005-06.
The motto behind the activities of the Math and the Mission,
as placed by Swami Vivekananda, is Atmano mokshartham
jagaddhitaya cha – “For one’s own liberation and for the welfare of
the world”. This is the essence of Sanatana Dharma viewed in the light
of the life and teachings of Sri Ramakrishna. To give a practical shape
to this idea Swami Vivekananda founded the Ramakrishna Math and
Ramakrishna Mission.
Ramakrishna Math and Ramakrishna Mission have, right from
their inception, accorded priority to service to the poor. For more than
one hundred years, the Math and Mission have been serving the people,
especially the poor and underprivileged masses in various ways. During
the year under review, the twin organizations made great strides in their
service activities, and our regular activities registered a steady and
significant growth.
NEW CENTRES
A list of new centres and sub-centres started during the year
2005-06 is given below:
In India
1. The Dilaram Bungalow at Vadodara, where Swami Vivekananda
stayed as a guest of Sri Manilal Jashbhai, the then Dewan of
Vadodara, was handed over to Ramakrishna Mission by the
Government of Gujarat. A new branch of Ramakrishna Mission,
named Ramakrishna Mission Vivekananda Memorial, Vadodara,
was started at the Bungalow.
2. A new branch of Ramakrishna Mission, named Ramakrishna
Mission Ashrama, Aurangabad, was started.
3. The newly created Ramakrishna Mission Vivekananda University
(formerly called Ramakrishna Mission Vivekananda Educational
and Research Institute) was made a branch centre of Ramakrishna
Mission.
4. Ramakrishna Math, Kadapa, till recently a sub-centre under
Chennai Math, was made a full-fledged branch centre of
Ramakrishna Math.
5. A branch centre of Ramakrishna Mission, named Ramakrishna
Mission, Kadapa, was also started at Kadapa.
6. The town centre under Ramakrishna Mission Vidyalaya,
Coimbatore, was made a new branch centre of the Ramakrishna
Math and named Ramakrishna Math, Coimbatore.
Outside India
1. Vedanta Society of Kansas City, USA, was made a sub-centre of
Vedanta Society of St. Louis, USA.
2. Three ashramas at Belo Horizonte, Curitiba and Rio de Janeiro
in Brazil were made sub-centres of Ramakrishna Vedanta
Ashrama, Sao Paulo.
3. Vedanta Centre of St. Petersburg, Florida, USA was made a
branch centre of Ramakrishna Math.
NEW DEVELOPMENTS
The following were the important developments in the activities
of the Ramakrishna Mission during the year 2005-06:
In the educational field several new educational units /
programmes were started as given below:
1. A deemed-to-be-university, Ramakrishna Mission Vivekananda
University (formerly called Ramakrishna Mission Vivekananda
Educational and Research Institute), established under

There has been an encouraging and persistent demand recently from the student of the
Ramakrishna-Vivekananda Literature for a fresh edition of this book, which had been long out
of print. We have taken this opportunity to include in the present edition some more
'Reminiscences' of Sister Christine, published a couple of years back in Prabuddha Bharata,
but not contained in the previous editions of the book.
An 'Appendix' is added at the end of the book. Strictly speaking, though they do not constitute
'Reminiscences', Miss Josephine Macleod's letters to her niece. Alberta Sturges, reveal the
force of the impact of Swami Vivekananda on Josephine's life and bring to light some hitherto
unknown facets of his personality and facts about his activities. These were compiled by
Professor Shoutir Kishore Chatterjee, of the University of Calcutta, from the original excerpts
made available to us by the Ramakrishna Vedanta Centre of England, and published in
Prabuddha Bharata earlier.
With the addition of these fresh materials, we earnestly hope that the book will be warmly
